Secondary Transition

Secondary transition planning is the process of preparing students for life after high school and includes planning for postsecondary education or training, employment and independent living.

Road Maps

The Transition Road Maps identify crucial tasks for specific age periods (14-16, 17-18, 19-21) during the transition years. In addition, they provide current information, contacts, and resource sheets. These “road maps” help students and their families to “drive their own car” through the transition years as effective self-advocates.

 

World's Best Workforce Plans

The World's Best Workforce (WBWF) is a plan that districts create annually to document steps they are taking to meet school readiness goals — including having all students attain career and college readiness before graduating from high school and having all students graduate from high school. Check out WBWF plans for each of our member districts.

 

Transition Resources

  • O*NET The O*NET database contains descriptors on almost 1,000 occupations covering the entire U.S. economy. O*NET information is used by millions of individuals every year to help people find the training and jobs they need.
  • National Technical Assistance Center for Transition Provides information, tools, and supports to assist multiple stakeholders in delivering effective services and instruction for secondary students and out of school youth with disabilities.
