Secondary Transition

Secondary transition planning is the process of preparing students for life after high school and includes planning for postsecondary education or training, employment and independent living.

Transition Resources

  • Disability Hub (E1MN) Employment supports for people with disabilities span multiple state agencies. E1MN brings things together to help the system make more sense. To get started, you can check out a E1MN chart.
  • O*NET The O*NET database contains descriptors on almost 1,000 occupations covering the entire U.S. economy. O*NET information is used by millions of individuals every year to help people find the training and jobs they need.
  • National Technical Assistance Center for Transition Provides information, tools, and supports to assist multiple stakeholders in delivering effective services and instruction for secondary students and out of school youth with disabilities.

 

CACR Plans

In Minnesota, Comprehensive Achievement and Civic Readiness (CACR) is a plan that every district updates annually to document steps they are taking to meet several goal areas — including:

  • All students graduate from high school.
  • All students are ready for career and college.
  • Prepare students to be lifelong learners.
